Should second molars routinely be replaced?
The answer is NO!
There are very few indications for replacing a second molar
   1.If the opposing 2nd molar is present and is good healthy condition and is also in correct position
   2.If the lack of cheek fullness is due to the absence of 2nd molar
   3.If the patient insists on replacing 2nd molar
The reasons why there is NO need to replace 2nd molars are
   1.The chewing efficiency on 2nd molar is only 5 to 8 percent compared to 80 percent of forces in First molar
    2.The bone height and quality is not favourable in 2nd molar area
    3.Resteicted access for treatment and also to maintain oral hygien
    4.occlusal force disturbances are more in 2nd molar area leading to severe stress on implants
    5.Imcreaed risk of implant failure in 2nd molar region
     6.increaswd probability of cheek biting after 2nd molar replacement
Because of this reasons..major implant centres and experienced dental implant dentists ..advise NOT to replace the 2nd molar ,unless there is a valid reason .
